How To Get Started Working With BeWell
New clients are invited to call the office at (814) 234-0785 to schedule their first appointment or to schedule a Free 15-Minute Consultation with Senior Nutritionist Monica Montag to find out how we can help you.
Once your Clinical Nutrition appointment is scheduled, we’ll send you an Intake Form consisting of a Health Appraisal Questionnaire and Food Diary, with instructions on how to fill them out. You’ll fill these out and send them back by the date specified before your appointment, so that your clinician can review them before you meet.
If you have recent blood work from your doctor, you can send in a copy of that as well. This will be analyzed from a functional nutritional perspective.
Whether your appointment is in person (currently suspended due to Covid-19), via Skype/Zoom, or by phone, your clinician will go over your assessment and ask any additional questions needed to find out as much as they can about what you’re experiencing and what you need help with.
She’ll also review the options for Functional Nutritional Testing and work with you to determine if testing is needed at that time.
By the end of your first clinical nutrition appointment, you’ll receive a Customized Support Program and Plan with recommendations for what to eat, targeted supplements, and other lifestyle guidelines.
You’ll be able to ask questions and get any clarifications on the recommendations either before you leave, or later via email, with your clinician.
When the appointment is complete, the office staff will help you fill your supplement order, which you can take home with you or, if your appointment was by phone, can be shipped to you.
You’ll also schedule your follow-up appointment, which will take place 2-4 weeks after your initial appointment.
At your follow up, you’ll report your progress and your clinician will update your Custom Support Program and Plan as needed.
Please remember that it’s vital that you send in your completed Intake Form, Health Appraisal and Food Diary, in advance of your first appointment, to allow you and your clinician to focus on creating your Custom Support Program and Plan.
